Types of physicals
School-entry exam (DH 3040)
Required for students entering a Florida public or private school for the first time, from Pre-K to 12th grade, including students moving from another state or country. It must be done within 1 year before enrollment.
Sports physical (FHSAA EL2)
Required by the Florida High School Athletic Association for students who play school sports. The form is valid for 365 days from the exam date.
Well-child check
Growth, development and health check-up for kids, with guidance for parents.
Annual physical for adults
A yearly wellness visit with history, exam, blood pressure and a screening plan, with lab tests when indicated.

School entry also requires a Florida Certification of Immunization (DH 680), which is a separate form.

Parent or guardian
Patients under 18 come with a parent or guardian, plus a list of medicines and allergies.
Florida rules in short
The school-entry health exam is required by section 1003.22 of the Florida Statutes, on Department of Health form DH 3040. It can be completed by a health professional licensed in Florida, or in the state where the student lived, who is authorized to perform general health exams.
The FHSAA preparticipation physical (form EL2) can be signed by the practitioners listed in section 1006.20 of the Florida Statutes, which include physicians, physician assistants and advanced practice registered nurses. Summer is the busiest season for school and sports physicals, so it pays to book early.

The FHSAA EL2 form is valid for 365 calendar days from the date of the exam.
When should the school-entry exam be done?

My child is transferring from another state. Do they need the DH 3040?
Yes. Florida requires it for the first entry into a Florida public or private school.
Is the vaccine certificate the same form?
No. School entry also requires the Florida Certification of Immunization (DH 680). Bring your child's vaccine records to the visit.
Do I need health insurance?
No. Physicals have a posted self-pay price. Card or Zelle accepted.
